Activity Locations

This template form will allow applicants to enter each location where their funded projects occurred.  The form is updated annually to include only the data that the NEA requests for funded projects.

You must include this page in the Final Report if you would like to easily create a locations report that is ready to go to the NEA.

Reference this short slide show if you report to the NEA so that you can create your grant opportunity in the way that will yield the best report to send to the NEA.

Questions cannot be added, modified, or removed. This form is updated annually to ask specifically for items requested by the NEA for locations reporting.
